To enter the Competition, you will need to complete an Entry Form (versions in Word or PDF formats). Please complete a separate form for each brand. There are two ways to enter:
(Please read General Rules first, then download the Entry Handbook*.)

1. Download Entry Form in Word format . (If having problem downloading, simply right-click over Word format link choose "Save target as" option.)
After you have fully completed the Entry Form on your computer, you may:

  • email it (as an attachment) to Competition Organizer, or
  • fax it to (562) 435-7118, or
  • send it to:
    Long Beach Grand Cru
    Attn: Executive Producer
    110 Pine Ave., #420
    Long Beach, CA  90802
    (DO NOT ship wines to this address)


    If you choose to fax or email your Entry Form, you must (postal) mail your (check) payment immediately afterwards, referencing the name of your winery/company used on the Entry Form and the date the form was submitted.
